Appointments may be scheduled by phone. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.
To assist us with billing your insurance efficiently and accurately, a copy of your insurance card will be taken prior to each visit. Also, please notify us immediately of any change in your insurance coverage. If you have any questions regarding your insurance or need assistance filing out your information, please call the office so we may assist you.
SELF-PAY: Payment in full is due at the time of service with cash, check, Visa, or Mastercard. A Quickpay discount may be applied when balance is paid in full.
THIRD PARTY: As a courtesy we will bill your Insurance carrier. If for any reason the Insurance refuses to pay or disputes the claim, the balance then becomes your responsibility.
MEDICARE: Our office will bill Medicare directly. You will be billed for the Medicare co-insurance and deductible unless you have a supplemental policy. Your supplemental policy will be billed following the guidelines set forth in this leaflet.
BCBS: It is our office policy to bill BCBS directly. You are responsible for any co-pays or deductibles at the time of service.
MASTER MEDICAL: If you have a Master Medical Policy, payment is expected at the time of service. Once payment has been received we will be happy to bill your insurance company.
BCN: Requires a referral from the patients PCP order to be seen at our office. The patient must present his/her card at the time services are rendered. The co-pay will be collected at the time of service.
WORKERS COMP/AUTO: If your visit is due to an accident related to work or auto you will need to obtain the following: 1) The name and address of the insurance company 2) The name and address of a person we may contact 3) A written authorization number. Please bring this with you to your appointment.
PRIORITY HEALTH HMO: The patient must present his/her Priority Health card at each appointment. The patient's PCP must supply us with a formal written authorization prior to services being rendered. We will bill Priority Health directly. All co-pays or deductibles will be paid at the time of service.
PPOM: The office will bill PPOM directly, the co-pay will be collected at each visit. In addition to the co-pay the patient will also be responsible for any deductibles or non-covered services.
MEDICAID: Straight Medicaid and McLaren Health Plan are no longer accepted in our office.
We do accept Health Plan of MI, Meridian and Priority Health Medicaid. If under 18 years old we do accept straight Medicaid and Molina. These types of insurances have been going through many changes. Some of these insurances cover podiatry to all memebers and others have stipulations you need to meet. We recommend you call your insurance to see if podiatry services are covered..
TRI CARE PRIME: Requires a referral from the patients PCP to be seen in our office.
NSF CHECK FEE: In the event a check is returned to the office, a fee of $25.00 will be added to the account.

If you would like to cancel an appointment, we ask that you provide us with at least 24 hours notice. A fee may be charged for a no-show or late cancellation, and patients may be discharged from our practice for repeatedly missing scheduled appointments.
