Appointments
Appointments may be scheduled by phone. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.
Financial
To assist us with billing your insurance efficiently and accurately, a copy of your insurance card will be taken prior to each visit. Also, please notify us immediately of any change in your insurance coverage. If you have any questions regarding your insurance or need assistance filing out your information, please call the office so we may assist you.
You are responsible for any co-pays at the time of service.
SELF-PAY: Payment in full is due at the time of service with cash, check, Visa, or Mastercard. A Quickpay discount may be applied when balance is paid in full.
THIRD PARTY: As a courtesy we will bill your Insurance carrier. If for any reason the Insurance refuses to pay or disputes the claim, the balance then becomes your responsibility.
MEDICARE/MEDICARE HMO: Our office will bill Medicare directly. You will be billed for the Medicare co-insurance and deductible unless you have a supplemental policy. Your supplemental policy will be billed following the guidelines set forth in this leaflet.
BCBS: It is our office policy to bill BCBS of Michigan directly.
BLUE CARE NETWORK: Requires a referral from the patients PCP order to be seen at our office. The patient must present his/her card at the time services are rendered.
HEALTH ALLIANCE PLAN (HAP) We do accept HAP.
MEDICAID: We do accept Meridian Medicaid and Priority Health Medicaid.
PRIORITY: We do accept Priority Health.
TRICARE PRIME: Requires a referral from the patients PCP to be seen in our office.
TRICARE: (for life, retired, remote) does not require a referral.
VETERANS COMMUNITY CARE: This requires an authorization from your VA PCM
WORKERS COMP/AUTO: If your visit is due to an accident related to work or auto you will need to obtain the following: 1) The name and address of the insurance company 2) The name and address of a person we may contact 3) A written authorization number. Please bring this with you to your appointment.
We DO NOT accept Ambetter, Priority NARROW Network, Straight Medicaid, Molina Medicaid, or McLaren Medicaid.
These types of insurances have been going through many changes. Some of these insurances cover podiatry to all members and others have stipulations you need to meet. We recommend you call your insurance to see if podiatry services are covered. We cannot guarantee payment from your insurance company.
NSF CHECK FEE: In the event a check is returned to the office, a fee of $25.00 will be added to the account.

Cancellation
If you would like to cancel an appointment, we ask that you provide us with at least 24 hours notice. A fee may be charged for a no-show or late cancellation, and patients may be discharged from our practice for repeatedly missing scheduled appointments.
